At Cook Vision Therapy Center, Inc., our goal is to provide the best in vision therapy so you and your child can enjoy learning to “See Happiness.” Payment is due at the time of service. We accept cash, checks, debit cards, Master Card, Visa, Discover, American Express, and Care Credit.
Insurance and Medical Coding Documentation
Cook Vision Therapy Center Inc. operates as an out-of-network provider. Because vision therapy is a highly specialized, individualized treatment program, this model allows Dr. Cook and Dr. Patel to prioritize clinical care over insurance constraints.
However, we fully support our patients in maximizing their health insurance benefits. Upon request, we provide detailed, itemized documentation (superbills) containing all necessary clinical descriptors, diagnostic codes, and standard medical coding strings. Many patients successfully submit these documents to their major medical insurance carriers for out-of-network reimbursement or apply them toward Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA).
Common procedural insurance codes utilized by our practice include:
- CPT Code 92060: Sensorimotor examination with multiple measurements of ocular alignment and binocular function.
- CPT Code 97533: Sensory integration training to enhance motor coordination, spatial orientation, and visual processing.
